Registration Information:
To register your child at Hillview Daycare Ministry, please contact our daycare office directly at 803-392-7197. After meeting with our director or assistant director for an interview, filling out all paperwork through our office and ProCare, turning in required Form 2900, and paying your registration fee, your child will be enrolled in the Daycare Ministry.
Items needed:
- General Record and Statement of Child's Health for Admission to Child Care Facility, Form 2900 (see below)
- Current South Carolina shot record (from your pediatrician's office)
- Non-refundable registration fee of $125.00 paid through the ProCare system (info provided by our office).
