Registration Information
Registration Information:
To register your child at Hillview Daycare Ministry, please follow the instructions at the bottom of this page. After meeting with our director or assistant director for an interview, filling out all paperwork through FACTS, turning in required Form 2900, and paying your registration fee, your child will be enrolled in the Daycare Ministry.
Items needed:
- General Record and Statement of Child's Health for Admission to Child Care Facility, Form 2900 (see below)
- Current South Carolina shot record (from your pediatrician's office)
- Non-refundable registration fee of $125.00 paid through the Nelnet FACTS Management System.
