Registration Information

To register your child at Hillview Daycare Ministry, please download the forms below, fill them out completely, and return them in-person to Hillview Baptist Church, 1910 Jefferson Davis Hwy., Graniteville, SC 29829 with your $125.00 non-refundable registration fee. After meeting with our director, filling out all paperwork, and paying your registration fee, your child will be enrolled in the Daycare Ministry.

Items needed:

• SC Early Childhood Registration Form (see below)
• General Record and Statement of Child's Health for Admission to Child Care Facility (see below)
• Current South Carolina shot record (from your pediatrician's office)
• Non-refundable registration fee of $125.00 in check (payable to Hillview Baptist Church), Money Order, or Cash form.
